Myelograms
What is a myelogram?

A myelogram is a two-part diagnostic procedure that can detect abnormalities such as a tumor, herniated disc, arthritis, spinal narrowing and/or nerve impingement of the spinal canal. 

How long does the procedure take?

Plan to be in our clinic for approximately 2 hours. This time is used for patient teaching, exam preparation, the myelogram procedure, CT scan and recovery time.

Does a myelogram hurt?

Most patients report a pressure sensation after the placement of the local anesthetic. Very few patients report any long-lasting painful sensations.

What are some of the possible side effects/complications?


Occasionally, patients will have a post myelogram headache. As with any procedure involving a needle there is a very small risk of infection or bleeding. Rarely there could be neural injury, headache, temporary numbness, weakness or facial flushing.

How do I prepare for the myelogram?

Food and fluids:
  • Do not eat solid foods for 3 hours prior to the exam.
  • Do not drink alcohol.
  • Drink clear liquids (water and juices) the day before the procedure.
Medication:
  • Certain medications can make you slightly more susceptible to seizures and should not be taken for 48 hours before or 24 hours after a myelogram. These include many of the anti-depressants, anti-psychotics and MAO inhibitors. Please check with your healthcare provider before this procedure to discuss the medications you are using.
  • If you have an active infection and are taking antibiotics, the medication course must be complete prior to the procedure, to ensure that no infection remains.
  • If you are taking blood thinners (Coumadin, Plavix, Ticlid), you will have to stop the medication prior to the procedure. Contact your healthcare provider before stopping any medication to determine if it is safe for you.
  • Take your regular medicine, such as antihypertensive medication, on the morning of the examination.
  • Bring a list of your medications to the clinic.
Pre-procedure process:
  • Bring a family member or friend to drive you home. We recommend refraining from driving for the rest of the day.
  • Wear comfortable clothes without metal fasteners, including zippers, buttons and snaps.
  • Arrive 15 minutes early to complete registration.
  • Bring with you: Your appointment form; insurance information, including your insurance card; valid photo i.d.
  • Please let us know about any of the following: Pregnancy, known or possible; allergies to medications, iodine or x-ray contrast; recent flu shot 
Image-guided injections

Our injections are image guided, which means that imaging equipment called fluoroscopy (a type of low-dose x-ray) is used to clearly view the area in real time, on a video monitor. This ensures exact placement of the needle during the procedure.

During the procedure

You will lie on your stomach on an x-ray table. A pillow will be placed underneath your stomach to help straighten your spine making it easier to place the needle. The area to be injected will be cleansed with an antiseptic solution. Local anesthetic will be applied to numb the area. A small needle will be placed into your spinal canal, and a sample of fluid may be obtained. Next, a small amount of x-ray contrast is injected through the needle. Some patients feel a slight pressure sensation or discomfort while the contrast is being injected. After the injection, x-rays will be taken using the fluoroscopy machine.

In some cases, a CT scan will follow the myelogram, which will take another 30 minutes. The images obtained from the CT scan provider greater detail about the internal structure of the spinal canal.

After the procedure

A summary of the findings from the procedure will be sent to your healthcare provider.

Unless directed otherwise, you may resume your normal diet, prescribed medications and activities, although rigorous activities should be withheld until the next day. In order to minimize the risk of a post myelogram headache, it is recommended that you remain lying down with your head slightly elevated for the rest of the day. Have someone stay with you for the first 12 hours in case you need help.

You may feel soreness at the needle insertion site for 2-3 days; you can use an ice pack to relieve any discomfort, up to 15 minutes per hour.

Drink plenty of clear liquids (water and juices) after your procedure and the day after. Do not consume alcohol until the day after your procedure. Medicines that were not allowed for 48 hours prior to the examination may be re-started 24 hours after the myelogram. Continue to take your regular medications.
